Avascular Necrosis (AVN)

Aseptic necrosis AVN Ischemic bone necrosis bone infarction  osteo necrosis as it is called refers to the cellular death of bone constituents brought on by a disruption in blood flow. Following the collapse of the bone structures there is discomfort a loss of joint function and long-term joint damage.

Overview


Diseases or severe trauma such as a fracture or dislocation that compromises the blood supply to the bone can result in osteonecrosis. Osteonecrosis can also develop in the absence of illness or trauma.


Causes


The following are some possible causes: 

  • Excessive drinking                                                                       

  • Abuse of intravenous or oral steroids 

  • Blood conditions such as sickle cell disease cause abnormal blood clotting fractures or dislocations near joints.  

  • HIV or the use of HIV medications 

  • Clotting disorders  

  • Chemotherapy or radiation therapy 

  • The illness is known as Gaucher disease in which harmful materials build up in the bones and particular organs.      

  • Systemic lupus erythematosus is an autoimmune disease that develops when the immune system of the body mistakenly attacks healthy tissue such as the skin joints and certain organs. 

  • Decompression illness caused by extensive deep-sea diving  


Symptoms


Initially, there are no symptoms. When bone loss worsens you may notice the following symptoms: 

  • Joint pain that may worsen over time and become severe if the bone collapses 

  • Discomfort that persists even when you are at rest restricted range of motion 

  • Groin pain occurs when the hip joint is impacted.

  • Limping if the disease affects the leg                            

  • Difficulty moving overhead if the disease affects the shoulder joint  

  • Worsening joint arthritic symptoms as the illness progresses



Ayurvedic View

Vata the predominant dosha involved is vata dosha. Due to following of vata kar ahara (food ) and vihara (activities), it enters into rakta vaha srotas (dhamani or sira-blood vessels), and rakta in those srotas undergoes Shoshana (depletion) by vata ruksha guna (dry quality) or there is obstruction of this srotas due to aggravated kapha, by these the dhatus (body tissues) undergo depletion which results in kriya hani (loss of function) in affected region and weakness, this is avascular necrosis. The Ayurveda approach is to bring dosha back to balance, help rehabilitate the affected area by ayurvedic panchakarma treatment, and following of healthy food and lifestyle to prevent the reoccurrence and complication of this disease.
