Uttara vasti

Uttara Basti is a specialized Ayurvedic treatment in which medicinal solutions are administered through the vagina and urinary passage (urethra). The term "uttara" translates to "above the anus," indicating the treatment's focus on the upper parts of the lower gastrointestinal and genitourinary systems.


 Importance


Uttara Basti plays a critical role in restoring balance within the body by alleviating the morbid doshas that contribute to disorders of the genitourinary system. It aims to return the doshas to their *prakruta awastha*, which signifies a normal and healthy balanced state regarding their qualities and functions. This treatment is especially indicated for a range of issues related to the genitourinary system, including but not limited to urinary discomfort and reproductive health concerns.


 Yantra Used for Uttara Basti


The procedure uses a specific instrument, or *yantra*, which consists of two primary components: the *basti netra* (nozzle) and the *basti putaka* (the urinary bladder bag).


 Vasti Netra (Nozzle)


The *vasti netra* is ingeniously designed to resemble the stem of jasmine or karaveera flowers (Nerium indicum). Its structure is broader at the center, tapering towards the end, akin to a cow's tail (go puccha). The sizing of the opening is specifically calibrated to allow a *sarshapa* (mustard seed) to pass through, ensuring that the medicinal solution enters the genital or urinary passage smoothly and effectively.


 Vasti Putaka (Urinary Bag)


The *vasti putaka* serves as the reservoir for the medicinal solution. This component is where the medicine is held before being administered through the *vasti netra*. Its design facilitates the careful and controlled delivery of the treatment.


Steps for Administration of Uttara Basti


1. **Patient Assessment**: A thorough assessment of the patient's condition is conducted to determine the most suitable medicine and dosage.

   

2. **Preparation**: Patients are advised to void their urine and stools prior to the administration of the medicine, ensuring a clean starting point.


3. **Preparatory Care**: The area for administration is prepared meticulously. A gentle *abhyangam* (oil application), is performed on the lower body, including the pelvis, hips, and groin, followed by mild hot fomentation to relax the muscles and prepare the tissues.


4. **Medicine Administration**: The medicinal solution is carefully filled into the *vasti putaka* and administered with utmost precision.


Indications


- **When Administered via the Urinary Passage (Mutra Marga)**:


  - *Mutraghata*: Conditions where there is an obstruction preventing normal urination.

  - *Mutrakricchra*: Difficulties experienced while passing urine.

  - *Ashmari*: The presence of renal calculi or kidney stones.


- **When Administered via the Genital Passage (Yoni Marga)** (for women only, starting from the fourth day of menstruation, once bleeding has ceased):


  - Prolapsed uterus.

  - Various menstrual disorders.

  - Enhancing fertility.


Precautions


- The nozzle or catheter must be inserted with caution; it should neither penetrate too deeply nor be positioned superficially within the genital or urinary passage. 


- Strict sterilization protocols must be followed before performing this procedure to prevent infections.


- If the administered medicine does not exit within 12 to 24 hours, it is crucial to take appropriate measures to expel any stagnated medicinal solution to prevent complications. 


Uttara Basti is a unique and effective Ayurvedic treatment that, with careful application and monitoring, can significantly improve the health of the genitourinary system.
